In order to be eligible, you must agree to each of the following. Please read each item and indicate agreement to all at the bottom.

I have previously attended at least one ESA Annual Meeting.

If selected, I will provide the time to communicate with my student throughout the summer. Anticipated commitment is 15 minutes per week.

If selected, I will attend the SEEDS orientation and dinner � 4 to 8 pm on Saturday, August 4, 2007; the SEEDS breakfast - 7 to 8 am on Monday, August 6, 2007; and, the Diversity Mixer � 6:30 to 8 pm on Wednesday, August 8, 2007. Cost of attendance will be covered by SEEDS.

If selected, I will provide the time to meet with my student each day during the Annual Meeting. Anticipated commitment is 60 minutes per day.
