Please read all of the information below carefully BEFORE completing this Meeting Registration Form.

The Ecological Society of America is acting as REGISTRAR accepting registrations from BOTH ESA and ISEM Members and non-Members. Be sure to indicate whether you are currently a Member of ESA or of ISEM in Section E. If you are currently a Member of both ESA and ISEM, please select only one category for registration purposes.

All Board Members, editors, speakers, presiders, presenters, and organizers are expected to register to attend the meeting.

Full payment for fees due MUST accompany your Meeting Registration Form. If you register using the Electronic Version of the Registration Form, your payment must be by  (MasterCard or VISA) ONLY.  *Credit card payments will not be processed unless the expiration date is provided. ESA does NOT accept American Express, Discover, or Diners Club. ESA also does NOT accept purchase orders. ESA Federal ID#: 37-0913411

All sections of the Meeting Registration Form marked with an asterisk (*) MUST be filled out completely. Please do NOT submit more than one Meeting Registration Form unless you are adding TRIPS, WORKSHOPS, OR TICKETED EVENTS to your original meeting registration.

PLEASE NOTE: Complete Section B on this form and pay the $25.00 Guest Registration Fee to register an adult who will accompany you at the ESA/ISEM Annual Meeting as a guest. This will provide him/her with LIMITED access to meeting programs and amenities. (See Registration Information in the Preliminary Program for details.) If an adult will accompany you at the ESA/ISEM Annual Meeting and he/she wishes to attend the full scientific program, he/she MUST complete a separate Meeting Registration Form and register to attend independently. You may register each child or youth who will accompany you at the ESA/ISEM Annual Meeting at no charge. (See Section C.)

Section B. Adult Guest Registration
To request Adult Guest Registration, please indicate the name of any adult guest (e.g., spouse, domestic partner, significant other, family Members, or other person age 17 or older) accompanying you at the ESA/ISEM Annual Meeting. You MUST register adult guests who will participate in a Scientific Field Trip and/or Ticketed Event and pay the $25.00 Guest Fee for each. Registered adult guests are entitled to LIMITED access to meeting programs and amenities as described in the Preliminary Program, but are NOT eligible to attend the full scientific program. Each adult guest who plans to attend the full scientific program MUST complete a separate Meeting Registration Form to register independently.

Section C. Child/Youth Registration
To request Child/Youth Registration, please indicate the name and age of each child or youth (infant through 16 years of age) accompanying you at the ESA/ISEM Annual Meeting. There is no charge to register a child or youth and registered children and youths may participate in Scientific Field Trips and/or Ticketed Events, except where otherwise noted, and .  Registered children and youths are entitled to the same LIMITED access to meeting programs and amenities as registered adult guests.

Section D. Special Requests
Child Care Services  
Optional child care services will be offered at the 2003 ESA/ISEM Annual Meeting and provided by Parent & Child, Inc. of Savannah, Georgia. ESA requests pre-enrollment of children no later than Monday, July 7 to ensure space availability and requires pre-payment of fees for all child care services. Two options are offered as described below.
Option A:

Infant and Toddler Care specifically designed for infants 3 to 24 months old, toddlers 2 to 5 years old, and pre-kindergarten children 5 years old, will be offered at the Savannah International Trade & Convention Center. Parent & Child, Inc. will provide service between 8 AM and 5 PM daily Sunday, August 3 through Thursday, August 7, and between 8 AM and NOON on Friday, August 8. Children may be enrolled for half-day and/or full-day care.
 

Option B:

Parent & Child, Inc. will operate its Savannah Summers “Just for Kids” program at an off-site location as a youth summer camp for children 5 to 14 years of age. Daily shuttle transportation will depart from the Savannah International Trade & Convention Center at 8:15 AM each morning and return at 5 PM each day Monday, August 4 through Thursday, August 7 and at NOON on Friday, August 8. Full-week and per-day enrollment will be offered.
 

To register your child/children for Infant and Toddler Care and/or Youth Summer Camp, please complete the Child Care Services Enrollment Form and Child Care Profile for each child registered. You will find a link to these and other meeting related Forms on the page that appears when you submit this Electronic Meeting Registration Form and on the Annual Meeting page on the ESA website (www.esa.org/savannah/).
Student Volunteer Opportunity
If you are an undergraduate or graduate student, you may apply to serve as a Student Volunteer at the 2003 ESA/ISEM Annual Meeting. Each Student Volunteer will be expected to provide a minimum of 14 hours of service at the meeting in exchange for a refund of his/her meeting registration fee after the meeting. Only a limited number of Student Volunteers will be accepted and these slots will be filled on a first-applied, first-notified basis. Please do NOT apply unless you will be available to provide 14 hours of service during the meeting. (Applicants who are selected will be notified in early July.)

PLEASE NOTE: Preference will be given to those who served as Student Volunteers at the 2000, 2001, or 2002 ESA Annual Meeting and/or those who are accepted as student field trip coordinators, van drivers, or session projectionists by Dr. Ron Carroll, Local Host Chair.

***To qualify for the Student Non-Member Rate, you MUST submit a copy of your valid student identification card to ESA Headquarters by fax at 202-833-8775 or by mail to 1707 H Street, NW, Washington, DC 20006. Those who register onsite, will be required to show a valid student identification card in order to receive Student rates.

Section F.  Scientific Field Trips

PLEASE NOTE: Each Scientific Field Trip participant (or the parent or guardian of each participant under 18 years of age) MUST sign an Accident Waiver and Release of Liability Form prior to trip departure. A pdf version of the form may be downloaded and printed from the ESA website (see at www.esa.org/savannah/ ) or a hardcopy may be requested from Onsite Meeting Registration staff or trip leaders.

We strongly urge you to read the Important Notes on enrollment, cancellation, and logistics that accompany the detailed descriptions of Scientific Field Trips in the Preliminary Program. The deadline for registering for these activities is Monday, July 7, 2003. Should your plans change, ESA will allow you to cancel if you do so in accordance with our Cancellation Policy. ESA RESERVES THE RIGHT TO CANCEL ANY TRIP FOR WHICH THE MINIMUM REGISTRATION IS NOT REACHED. Coweeta LTER/UGA Institute of Ecology
Trip departs from UGA in Athens. Participants must arrive at in Athens by 10 PM on Friday, August 1 (Overnight accommodations included in fee)
Departs on Saturday, August 1 at 7 AM from UGA; Returns on Sunday, August 3 by 5 PM to the SITCC.
Fee includes two nights lodging, two lunches, soft drinks and water, handouts, and transportation from the University of Georgia (UGA) to Savannah. Breakfast and dinner on Saturday, breakfast on Sunday, and transportation to UGA on Friday, August 1 are at your own expense.

Jones Ecological Center - The Fire Forest: The Longleaf Pine Ecosystem
Departs on Saturday, August 2 at 8 AM; Returns on Sunday, August 3 at 7 PM
Fee includes transportation, overnight lodging, meals, and handout materials.

Overnight Trip to Stephen C. Foster State Park in the Okefenokee Swamp
Departs on Saturday, August 2 at 7 AM; Returns on Sunday, August 3 at 6 PM
Fee includes transportation, lodging, two lunches, and admission to the State Park. Breakfast and dinner and optional  boat rental for water trips are at your own expense.

Low Country Tour with Dutch-Treat Dinner at Crab Shack

Departs Saturday, August 2 at 5:30 PM; Returns at 9 PM with stops at several official lodging locations.
Fee includes transportation with en-route interpretive narrative and stops in the Low Country marshes and Tybee Light House. Dinner is at your own expense.
