CHILD CARE SERVICES

General Information
As in past years, ESA will underwrite a portion of the fees for child care services in order to be able to offer them at very affordable rates. If you have children who are young, school age, or up to early teens, we invite you to bring them with you to the 2005 ESA-INTECOL Joint Meeting and take advantage of these services. Not only will your child be able to receive quality care, have fun, and enjoy companionship and stimulating play while you attend sessions, but also you will be able to give full focus to the presentations. This can also be a courtesy to other attendees and presenters as many prefer not to have the distraction of children in the meeting rooms. 

Two types of Child Care Services will be offered:
Day Care for children 3 months to 4 years of age, and Day Camp for children 5 years to 15 years of age. Both types of child care will be provided by La Joutheque of Montréal Canada.

ESA requires pre-enrollment of all children to ensure space availability and pre-payment of fees for all child care services. For each child enrolled, ESA also must receive a profile of the child’s pertinent health and development information, emergency contact information (e.g., for the child’s health care provider, a parent or guardian, and an alternate contact), and signature authorization/permission as required by both ESA and La Joutheque. In addition, ESA requires that parents complete an Accident Waiver and Release of Liability Form for each child they enroll for either Day Care or Day Camp. 

The forms required to enroll your child(ren) for child care services are available in electronic and/or PDF format in the ESA website at http://www.esa.org/program/  forms.html. The completed Child Care Services Enrollment Form and fee payment, Child Profile Form, and Accident Waiver and Release of Liability Form must be received at ESA Headquarters by July 18, 2005. Requests for onsite enrollment will be accepted on a space available basis ONLY. (For onsite enrollment, forms will be collected and processed at the Meeting Registration Desk.)
